Note: All Templates marked with an asterisk require our integrated Giving Software.
Express Giving Form
The Express Giving Form already includes all necessary fields. Any active funds you have created will be available to add to the dropdown.
Giving Fund List Form
The Giving Fund List Form template will show all funds set up in Fund Management. You can hide irrelevant funds via the Field Set Properties. You will also be able to add images to each fund via Fund Management (by default, a blue mountain image will appear next to any funds without an image uploaded). Donors can enter a total gift amount at the top of the form, and then select Divide Among All Funds or Apply To All Funds. Alternatively, donors can select differing amounts, separate recurring frequencies, and even start dates for each fund.
Note: The image size should be no larger than 1526 x 1145 pixels or 5 MB
Second Collections (Catholic Client Feature)
This template will create a Giving form populating all active Second Collection Funds. Although the best practice is to add the Second Collections Field Set to your Primary Giving Form, if you'd prefer a dedicated Second Collections form, this template is for you.
Contact Form
The Contact Form template provides a simple way to gather basic information from anyone who needs to contact your ministry/organization.
Event Registration
The Event Registration template makes it easy to register someone for any event you're hosting. It is already set up to allow adding another registrant, so multiple people can register using just one form.
Children's Event
The Children's Event template includes commonly used fields for gathering important information about a child, such as the parent/guardian's name, emergency contact, medical information, etc. It also allows you to register multiple children using the same form.
Subscribe Form
The Subscribe Form template allows people to request subscriptions to newsletters, bulletins, etc.
Prayer Request Form
The Prayer Request Form template allows people to submit a prayer request for the staff/prayer team to receive and pray for them. It includes conditional formatting already set up! So when someone selects Yes for Would You Like Someone From The Team To Contact You?, the phone number and email fields will appear.
Online Attendance
The Online Attendance template can be made available to those who weren't able to attend in person, so they can let you know they attended |online that day. It also allows you to give online on the same form.
