Forms: Contact Info Fields

Use Contact Info Fields to ensure name, email, phone number, and address are collected and stored in a standard format

Note: The standard formatting enforced by Contact Info Fields ensures that demographic information works correctly when combined with profile updates, Field Sets, or Workflows.

Along the right side of the form editor window are standard controls to move the field up or down within your form, duplicate the field, configure field properties, or delete the field from the form. 

Form Field actions from left to right:

  • → Grip and drag to move the field on the form
  • → Clone or duplicate the field
  • → Edit (Configure) the form field properties
  • → Delete the field from the form

 

Available Fields

Person's Name

Collect the First Name and Last Name of the registrant or some other contact. The Person Field Set includes this field by default.

Email

Easily collect correctly formatted email addresses for export or import to the database when mapped through a workflow.

Phone Number

Collect a phone number in the standard format of any country. 

For numbers outside North America, users can click the phone and flag icons on the left-hand side of this field to display a country code dropdown menu. Quickly select a country by scrolling or typing to search. 

This field also applies the correct phone number formatting when exporting or importing data into a workflow.

Address

The address field adds multiple preformatted fields in a single step. By default, the United States format is selected, which adds fields for:

  • Address Line 1
  • Address Line 2
  • City
  • State
  • Country (defaults to United States)
  • Zip Code

The Country Field always displays as a dropdown, allowing users to select a different country as needed.

Change the Default Country

The Phone and Address fields allow you to set the default country to something other than the United States. Doing so may change how certain fields are displayed, based on the selected country. 

For example, if the Address Field is set to Canada, the State and Zip code fields are replaced with the Province and Postal Code fields.

To change the default country:

  1. Click the Gear (configure) icon to edit the Phone or Address fields.
  2. Under the General Tab, use the Default Country dropdown to select a country.
  3. Click Done, then Save or Publish your changes.

Note: Changing the default Country must be done on a form-by-form basis.
